It creates a log of the items, with information explaining why HijackThis flagged them in the first place, and you can decide if they are malware or not. Just check off the ones you want to delete and click on the "Fix Checked" button.
So the beauty of this software is that it doesn't look for specific spyware programs that already exist. Rather, it hunts for the kinds of entries and activities that would be used to invade and infect your computer. You'll find a subtle variation in how it works compared to other programs, since it doesn't have to know about all the different malware programs out there.
On CNET.com, editors reviewed HijackThis and gave it high praises with the software earning a perfect rating, giving it five out of five stars. There was a slightly lower score with the SnapFiles review, and it only got a three of five score from PCWorld. Their complaints aren't as much about the operations of the software, as they praised and lauded those aspects; the only real issues they had were that they were difficult to understand by people who are unaware of how to look for malware and registry keys, etc.

Just stay away from the 'Fix Checked' button until you're confident that you know what you're about to zap.
In general, this software is very highly regarded. Exercised with great care, HijackThis appears to be very useful for helping to get rid of most spyware and other malware installed on a computer.
